It's a hand on a stick. You see him do some random blobbing, but all the detail is done by a hand on a stick, you can see that the position of the hand never changes and at 43 seconds if you pause at exactly the right time (it's sped up so very hard to get) you can see thin air between the hand and the bottom of the screen, showing that it's not connected to an arm.
At the beginning of the painting, if you look the first thing he does is blot really hard on one spot and smudge it around, then when it cuts to the other bit, there's loads of precise little dots.

well if you look closely you can see a difference
when he is painting or suppose to be painting he is using only his index finger but when they show the whole boy he is using like his whole hand
and he is looking to someone like "am I doing this right?"

this is legit and despite the belief that it isn't geniuses actually exist and this kids is on of them
